New Patient Registration

child_plasterIf you live in our practice area and would like to register with us please ask our receptionists.

If you need to see a doctor urgently, please inform the receptionist and they will make arrangements for you to be seen.

When registering please bring photographic ID such as your passport or driving licence. - This has been currently suspended due to Covid 19. 

You will be asked to complete a registration form and a new patient questionnaire, which will assist us in providing medical care while your medical records are transferred from your old practice to this one as this can sometimes take up to three months.

You will be asked to make an appointment with our Healthcare Assistant for a routine new patient check-up on completion of the registration forms. If you take regular medication you will also be asked to make an appointment with a GP.

If you move out of the practice area, you will need to register with a new practice as soon as you move. We can provide information about changing practices.

You will need to complete BOTH of the forms below to register.

You can print off a registration form and questionnaire using the links below, fill it out and return to the surgery via our black post box located on the outside of our building near the front door, via post or by email to ORK.dounbysurgery@nhs.scot

Please note, if you are a resident of Rousay, Egilsay or Wyre you will need to register with the Rousay Surgery directly.
